What to include

  • Technical / submissions: form slug or endpoint URL, time in UTC, browser or client, HTTP status or error text, webhook delivery ID if relevant.
  • Billing: account email, plan name, and invoice or charge date if you have it.
  • Enterprise: company, expected monthly submissions, regions, security or compliance requirements, and target go-live.
